Job description: Wellness Coach/Case Manager

Responsibilities:

• Engage people with HIV/AIDs; mental illness; and Substance use disorder and assist them in meeting the demands of community living and maintaining their housing by teaching them the necessary skills, and linking them to services that will support their recovery, independence and self-determination
• Document engagement in bi-weekly-monthly progress notes.
• Complete service plan reviews; WRAPs, SNAPs and Functional assessments for all residents
• Provide Case Management for residents, engage residents in setting rehabilitation and housing support goals, and collaborate with them in developing a Housing Support Plan where indicated.
• Quarterly Apt inspections-Regularly assess, correct and report apartment and personal living space inspections conditions as assigned

Requirements:

• Bilingual (English/Spanish) preferred
• Bachelor’s Degree in human services or related field required
• A minimum of two years’ case management experience
• Computer literate in word processing and excellent writing capabilities to develop treatment plans and progress notes

Job Types: Full-time, Contract, Temporary

Pay: $23.00 per hour

Expected hours: 35 – 40 per week
