Medical Social Worker:

Blize Healthcare is seeking a Medical Social Worker to support patients and families as they plan for the emotional, social, and financial challenges related to illness. This role plays an important part in promoting patient well-being, supporting care transitions, and having access to needed resources.

Responsibilities:

• Assess the psychosocial needs and functioning of patients and families.
• Provide individual or group counseling for emotional, behavioral, or social concerns.
• Coordinate care and connect patients with community resources and support services.
• Advocate for patient needs and assist with accessing benefits.
• Support patients and families during transitions from hospital to home or other care settings.

Requirements:

• Master’s degree in Social Work (MSW) from an accredited program.
• CPR/BLS certification.
• Valid driver’s license.
• Ability to lift up to 50 lbs as needed.
