The New American Association of Massachusetts (NAAM) is a non-profit organization which provides social support services to refugees and immigrants residing on the North Shore. The core services include: ESOL classes, employment assistance and job coaching, skills training, case management, after-school and summer programming for youth and children K-12, citizenship assistance and civics/history classes.

Position Overview

The Wellness Clinic Enrollment and case management specialist conducts enrollment and provides short-term case management to newly arrived refugees and immigrants experiencing stress, trauma, or emotional challenges. This position is non-clinical and focuses on connecting clients to essential services, completing wellness assessments, individualized wellness plan and assisting with care coordination in collaboration with the STAR Wellness Clinic team. This role will work closely with the wellness clinic director and other NAAM department staff with a focus on timely and appropriate case management and psychosocial care coordination and referrals to community providers. This role requires a good knowledge of (and willingness to learn) basic clinical concepts and an understanding behavioral health and trauma informed care.

Key Responsibilities

Conduct initial enrollment and follow up outreach individuals and families that have been referred to the wellness clinic using telephone, video , text message, or in-person.

Meet with clients to provide case management and monitoring progress of assigned clients in case load, with a focus on managing individualized goals, and connection to community resources and providers to improve wellness.

Facilitate access with families and children on a range of issues providing them assistance and access to schools, health and mental health care, food resources, service providers, volunteering groups, educational opportunities, job resources, legal assistance, social benefits (such as DTA, SNAP, MassHealth, childcare, housing, immigration assistance, mediation, domestic violence prevention programs, divorce/family issues), and other services as needed.

Support individuals and families by addressing logistical barriers, scheduling challenges, childcare needs, etc., that would inhibit them from attending their appointment.

Conduct initial and follow up STAR–Program Data Points (STAR-PDP) form of referred individuals and families and support the tracking of benchmarks of progress, including short term goal completion.

Provide in office and community-based support and consultation to support clients with accessing necessary social, medical, behavioral support services and programs to support their wellness

Engage interpreters and translators if needed to facilitate communication between clients and service providers in

Maintain confidentiality and up-to-date documentation, including, but not limited to case files, client logs on electronic system, referral status, outcomes, and ongoing case management as required by NAAM and ORR

Maintain regular communication with supervisor through emails, phone calls and participate in case review meetings

Track short-term goals and complete required documentation

Collaborate with interpreters as needed

Attend initial and continuing education ORR training programs and wellness clinic activities. including self-directed reading and in-person and online learning.

Competencies/Skills:

English language proficiency is required.

Proficiency in Spanish and/or other language fluency [Pashto, Dari , Swahili, French, Haitian-Creole, Russian,] desirable

Experience and knowledge of laws, benefits and services available for refugees and immigrants.

Cultural sensitivity and knowledge of clients’ backgrounds and needs.

Ability to work with diverse constituencies and act as mediator between cultural groups.

Strong organizational, interpersonal, attention to detail, time management, multi-tasking, problem-solving skills and ability to follow directions.

Strong oral and written communication skills and ability to set personal boundaries

Familiarity with community resources and basic behavioral health concepts

Must have reliable transportation to get to other sites and meetings as needed

Strong collaborative and negotiation skills including ability to establish and maintain effective relationship with providers, administration, and staff.

An ability to understand and communicate the impacts of structural and institutional oppression on healthcare on various marginalized identities such as gender, sexuality, race, and immigration.

Education/Experience:

Associates, Bachelor’s or master’s degree in health sciences, social work, sociology, human services or related field preferred;

Two years of human service experience with refugees and immigrants in the United States.

Experience and/or strong knowledge of behavioral health, mental health and/or trauma survivors is highly preferred.

Demonstrated Tech proficiency ( Zoom, Google Docs, Microsoft 365, Salesforce, electronic health records and Sharepoint)

Requirements:

The physical demands described here represent those that an employee must meet to perform the Key Responsibilities of this job successfully. While performing the duties of this job, the employee is required to talk and hear regularly. The employee must be able to remain in a stationary position 40-80% of the time. The employee needs to move inside and outside the office to access file cabinets, office equipment, meet with community partners at different sites. The position requires occasionally lifting office products and supplies up to 50 pounds and traveling to various locations for meetings and client case management.
