Job Description\n

American Traveler is seeking an experienced RN Case Manager for a home hospice assignment requiring a VA or Compact RN license and recent hospice case management experience.

\n \n Responsibilities\n

• Home hospice case management covering patient residences, long-term care, and assisted living facilities
• Day shift schedule, 5x8 hour shifts from 8:00 to 16:00, with every other weekend required
• Assignment length is 13 weeks
• Weekly interdisciplinary team (IDT) meetings are part of the workflow
• Floating may be requested based on facility needs
• Responsibilities include symptom management and end-of-life support for hospice patients in various residential settings
• RTO requests must be kept to a minimum, with no more than 7 days off and no full calendar week off during the assignment
• First day compliance requires submission of SSN and copies of identification
• Team environment with weekly IDT meetings offering collaborative support

\n \n Requirements\n

• Active VA or Compact RN license required at time of consideration
• Minimum 1 year of experience in hospice or home care case management within the past year
• EPIC charting experience required
• At least one supervisory reference required for consideration
• Locals are accepted at the same rate
• Candidates who were prior permanent employees must have a 6-month gap before returning as travelers
